Oakgrovemidwife.info was the online home of Oak Grove Midwifery, LLC (OGM), the practice of Licensed Midwife Stacy Vandenput, CPM. From 1990 to 2022, OGM provided midwifery care, childbirth education, doula services, and health resources to families throughout Green Bay, Northeast Wisconsin, and Michigan’s Upper Peninsula.
The site now serves as a legacy record of that practice and is being curated for archival preservation. While this process is underway, the site remains private, and its materials are not currently available for active use.
For Former Clients and Newborn Patients
- If you were an OGM client between 2015 and 2022, you may be able to access your electronic records directly through the ClientCare portal.
- If you are an individual born under the care of OGM during those years, you may also request access to your birth or patient records using the contact form on this site.
- For clients and patients served before 2015, records are maintained in physical storage and must be retrieved and digitized before they can be released. Please allow up to four weeks for processing these requests.
